ACTIVE BYU PERSONNEL
HEALTH AND WELFARE BENEFITS (INSURANCE)
Health and Welfare benefits are coordinated through Deseret Mutual Benefit Administrators (DMBA) in Salt Lake City. BYU-Provo personnel are required to be hired as regular full-time employees (i.e., 40 hours per week) to be eligible for Deseret Mutual core plans and supplemental plan insurance benefits. Special medical plans are available for temporary or part-time non-student employees on J-1 Visas.
Available core plan coverage for eligible employees includes medical, dental, Group Term Life, Occupational Accidental Death & Dismemberment and Disability insurance. Spouses and unmarried dependents can participate in the medical, dental and Group Term Life. Details are available in the DMBA handbook.
Supplemental insurances for eligible employees, spouses and unmarried dependents include Supplemental Group Term Life, 24-Hour Accidental Death & Dismemberment, and long-term care coverage.
FINANCIAL BENEFITS PLANS
Personnel eligible for the Health and Welfare benefits may also participate in the DMBA Flexible Spending Account (FSA) plan. Annual renewal is required during the open enrollment period.
Full-time personnel (excluding Visiting Faculty and Post Docs), part-time benefited faculty, and part-time Category II staff personnel may participate in the Deseret Mutual Master Retirement plan. Participants must be age 21 and work 1,000 hours in a twelve-month period to trigger eligibility requirements.
These same personnel may participate in the Deseret Mutual Thrift plan with matching provisions.
All university personnel are eligible to participate in the TIAA-CREF Supplemental Retirement Annuity (SRA) plan. Regular full-time personnel may also send money by payroll deduction to a TIAA-CREF Roth IRA plan.
BYU SPECIFIC BENEFITS PLANS
Full-time personnel, part-time faculty, and part-time Category II staff employees may have a BYU ID card with activity privileges including use of Wilkinson Student Center facilities, a discount at the Bookstore, campus parking, library access, and the purchase of tickets for athletic and cultural events at reduced prices. Spouses of regular full-time personnel and their single children between ages twelve and thirty may also have an ID card with activity privileges. See the Identification (ID) Card Policy for details on eligibility and activity privileges.
Tuition benefits are available to regular full-time personnel, their spouses and dependent children. Specific benefits and limits are described in the University Personnel Tuition Policy.
RETIRED BYU PERSONNEL
HEALTH AND WELFARE BENEFITS (INSURANCE)
Health and Welfare benefits are coordinated through Deseret Mutual Benefit Administrators (DMBA) in Salt Lake City. BYU-Provo personnel are required to be full-time employees age 55 or older with ten or more years of Deseret Mutual Master Retirement Plan benefit credit to be eligible for Deseret Mutual retiree medical and dental plans. A premium advantage is available for those with twenty or more years of service. Full-time employees of BYU who were age 55 or older on September 1, 1996, must have at least five years of benefit credit to be eligible. Eligible retired personnel under age 65 may choose one of the Deseret Mutual regular medical insurance plan coverages. Eligible retirees age 65 or older may choose one of the Medicare Advantage private fee-for-service plans offered by Deseret Mutual. Coverage may be selected for retirees, spouses and unmarried dependent children under age 26.
Dental plan coverage may be elected for retirees, spouses and unmarried dependent children at the time of retirement.
Group Term Life coverage of $12,000 may be continued into retirement by eligible retirees along with a limited amount of Supplemental Group Term Life coverage. Spouses and unmarried dependents may also apply for Supplemental Group Term Life coverage at the retirement of the eligible employee.
No accidental death & dismemberment insurance, nor disability insurance is available through Deseret Mutual after retirement.

FINANCIAL BENEFITS PLANS
Deseret Mutual Master Retirement Plan benefits are available for payout at retirement at age 55 or older. A number of annuity payment options are available.
Retirees may no longer contribute monies to their Deseret Mutual Thrift plan or their TIAA-CREF Supplemental Retirement Annuity (SRA) plan. A number of payment options are available for participants after age 59 ½ or after retirement. (Retirees who return to work at BYU may save in TIAA-CREF.)
No Flexible Spending Account (FSA) is available through Deseret Mutual after retirement.

BYU SPECIFIC BENEFITS PLANS

Activity privileges are not extended to retirees from part-time employment or to retirees from BYU-Hawaii, BYU-Idaho, or other CES institutions. Tuition benefits are available to eligible retirees, their spouses and dependent children. Specific benefits and limits are described in the University Personnel Tuition Policy.
Regular full-time personnel who terminate employment after age 55 or older with five or more years of full-time BYU-Provo employment are considered eligible retirees for purposes of the BYU specific benefits plans.
SURVIVING SPOUSES AND CHILDREN OF DECEASED UNIVERSITY PERSONNEL OR RETIREES
HEALTH AND WELFARE BENEFITS (INSURANCE)
Medical/dental insurance may continue at no cost for a one-year period for a surviving spouse and unmarried dependent children under age 26 if they were enrolled in the programs at the time of the employee's death. Details regarding eligibility to continue medical/dental insurance after the one-year period are available from BYU Benefits Services or from DMBA.
Group Term Life insurance and 24-Hour Death and Dismemberment insurance are not available to surviving spouses or dependent children following the death of an employee/retiree.
Surviving spouses of retirees may retain, without proof of insurability, the amount of Supplemental Group Term Life (SGTL) insurance coverage they had in effect at the retiree's death ($5,000 - $25,000, with a maximum of $15,000 at age 75). Surviving spouses of active employees may enroll in SGTL (($5,000 - $25,000) without proof of insurability if they had at least that much coverage in effect at the time of the employee's death. Surviving dependent children may keep $5,000 of coverage without proof of insurability if they were enrolled for $3,000 or $5,000 in SGTL at the time of the employee's or retiree's death. Dependents enrolled for $10,000 may continue at that level. Surviving spouses and children not enrolled in SGTL at the time of the employee's or retiree's death may apply for $5,000 of coverage by proving insurability. They will be billed by DMBA for the premiums.
Upon receiving notification of the death of an employee or retiree, DMBA sends information and enrollment papers to the surviving spouse for the plans in which eligibility continues.
FINANCIAL BENEFITS PLANS
Master Retirement Plan eligibility is defined in the plan and is connected to eligibility at the time of death. Details about the retirement plan are available from BYU Benefits Services or from DMBA.
Ownership of the savings plans (Deseret Mutual Thrift Plan, Deseret Mutual Tax Sheltered Annuity Plan, TIAA-CREF Retirement Annuity Plan, TIAA-CREF Supplemental Retirement Annuity Plan, and TIAA-CREF Roth IRA Plan) will be transferred to the beneficiaries listed on file at DMBA and TIAA-CREF. If no beneficiaries are listed, the funds will be disbursed by process of law.
BYU SPECIFIC BENEFITS PLANS
Surviving spouses remain eligible for activity privileges as outlined in the Identification (ID) Card Policy. Dependent children remain eligible for ID card privileges as outlined in the Identification (ID) Card Policy until the age 30 or until they marry, whichever occurs first.
If the surviving spouse and children were eligible for tuition benefits at the time of the employee's/retirees death, eligibility may continue under the guidelines provided in the University Personnel Tuition Policy. Please consult the policy or contact BYU Benefits Services for more information.
