Tuition Benefit for Independent Study

For use by full-time regular faculty, staff and administrative personnel, to register for Independent Study courses without being required to pay the cost of tuition. Personnel may only register for a TOTAL of 6 credit hours per semester (or a combined total of 6 credits per term with no more than 4 in a term).

Request for Employee-Vendor Approval

To be completed by all faculty and employees who proposes to sell goods or services to Brigham Young University.

Supplemental Compensation (Student and part-time employees)

Part-time supplemental compensation is compensation paid in addition to regular wages for work which is performed apart from and in addition to the employee filling his/her regular University assignment.

Sick Leave Assistance

Sick Leave Assistance – Full-time administrative and staff employees may donate vacation hours to other eligible employees in the college/department/division in case of a serious illness or medical emergency. Contact the EEO Manager at 422-5895 for approval.

A Two-Part Process

Supplemental compensation is any compensation paid in addition to base pay for professionally-related services rendered outside of regular assignments and working hours. This information is intended for administrative employees only.

Reference Form

This Reference Form may be used by Hiring Managers and other employees to submit two references for each applicant being considered for hire. For help in completing the reference checks, please see "Reference Checks" under "Hiring Tips and Guidelines - Non-Student." When the form is completed, it may be faxed to 422-0209 or be brought into the Employment Services Office, D-70 ASB.

Approved Part-Time Request Form and Memo

All requests for new approved part-time positions must go to the Human Resource Committee (HRC) for approval. Use this form to have your request presented to the Human Resource Committee. You will be notified of the Committee’s decision by phone or email a few days after the meeting. If you have any questions, please call me at 2-8650.

Student Appeal Form

The Special Request Form can be used to appeal to work more than the alloted 20 hours or to be eligible to work with less than 6 credit hours.
