Request a Waiver - Instructions

All of the following steps must be completed before your waiver will be processed:

Click on the Request a Waiver link on the Off-Campus Housing Homepage
Log in using your Net ID and password
Fill in Current Phone Number and Address of Non-compliant Unit (place you want to live)
Click on the appropriate waiver choice, either, "I live with family", defined as: (parents, grandparents, married brother, sister, aunt or uncle) or "General Waiver" (for all other circumstances)
Completely fill out the online application (All boxes must be filled in)
Print a copy of the waiver application
Sign the waiver application ("I Live with Family" waiver must also be signed by relative)
Print a copy of the Residential Living Standard Agreement ("General Waiver" only)
Fill out the Residential Living Standard Agreement and get signatures of all occupants, including yourself
Bring, FAX (801) 422-0182, or mail these signed documents to the Off-Campus Housing Office, C-141 ASB Provo, Utah 84602

You will receive confirmation e-mails when:

Your online application has been successfully submitted
We have processed your application and a decision has been rendered. The waiver process is not complete until you have received this e-mail
